Diabetes.
Uncontrolled diabetes can cause sweet-smelling urine. This is due to increased sugar (glucose) in the urine, which occurs when the blood sugar level is too high and the kidneys cannot filter it all. If you notice that your urine has a sweet or fruity smell, especially if you are thirsty and urinate frequently, this may be a sign of undiagnosed or poorly controlled diabetes. Untreated diabetes can have serious health consequences, so you should see a doctor as soon as possible.

Pharmaceutical and supplementary products.
Some medications and supplements can change the smell of urine. Antibiotics such as penicillin and sulfa drugs, as well as supplements such as vitamin B6, can cause urine to smell. For example, high doses of vitamin B6 can cause urine to have a strong, almost fishy odor. Although this side effect is usually harmless, it can be disturbing. If the smell bothers you after taking the medicine, consult your doctor.

Liver disease.
Liver problems can cause changes in the smell and color of urine. If the liver is not functioning properly, excess bilirubin accumulates in the body, producing thick, foul-smelling black urine. This can occur in diseases such as hepatitis and cirrhosis. Other symptoms of liver disease include yellowing of the skin and eyes (jaundice), fatigue, and abdominal pain. If you notice a combination of these symptoms, it’s important to seek medical attention, as liver problems may be serious.
